Please check the Date of Service, which is located above your address on the right-hand side of your card to ensure you get the correct information. If you will open your card up, there are Disqualifications and Exemptions on the backside. If any of the Disqualifications are applicable to you, please circle the appropriate one and sign and date your card and mail back to the County Clerk. If any of the Exemptions apply, and you wish to claim the Exemptions, please circle the appropriate one and sign and date your card and mail back to the County Clerk. There is no need for you to call if you are claiming a Disqualifications or an Exemptions just return your card to the Wise County Clerk.

Failure to Answer Summons and Provision of False Information

A person who fails to comply with this summons, or who knowingly provides false information in a request for an exemption or to be excused to from jury service, is subject to a contempt action in accordance with Government Code, Section 62.0141 and Government Code, Section 62.111. See also Code of Criminal Procedure, Article 35.01.
